Requesting a Birth Certificate

Adult adopted persons in Illinois have the option of requesting non-certified copies of their original birth certificates. State law provides allowances for adult adopted persons and birth parents regarding these documents.

For Adopted Persons

If you were born in Illinois before January 1, 1946, you may request a non-certified copy of your original birth certificate, beginning May 21, 2010, by filing a special form with the Illinois Department of Public Health. The filing fee is $15.00.

If you were born in Illinois on or after January 1, 1946, you may request a non-certified copy of your original birth certificate, beginning November 15, 2011, by filing a special form with the Illinois Department of Public Health. The filing fee is $15.00.

If you are a surviving spouse or surviving adult child of an adopted person, you may request a non-certified copy of that person’s original birth certificate.

For Birth Parents

If you are the birth parent of an adopted person who was born in Illinois before January 1, 1946, you may not request that your identifying information be deleted from the non-certified copy of your child’s original birth certificate.

However, you may express your preferences regarding contact, including a desire not to be contacted, by signing up with the Illinois Adoption Registry and Medical Information Exchange and filing a written statement. There is a $15 fee associated with the registry. This fee will be waived for birth parents who voluntarily complete a two-page medical questionnaire on genetically-transmitted diseases.

If you are the birth parent of an adopted person who is over the age of 21 and was born in Illinois on or after January 1, 1946, you may request that your identifying information be deleted from any non-certified copies of your child’s original birth certificate that are issued during your lifetime.

To remove your identifying information, you must complete a Birth Parent Preference Form and select Option E before November 1, 2011. Other options are available for birth parents who would welcome contact with their child, either directly of through a third party.
